This is the sixth edition of a well-known book now co-edited by Ronald D. Miller and Manuel C. Pardo who replaces Robert Stoeling. With some 60 contributors largely but by no means exclusively, drawn from the University of California, San Francisco, the editors have to be congratulated on achieving a uniformity of style and level of academic content. The book is aimed at students of anaesthesia at all levels of training and is laid out in a way which takes the reader through the subject in a well structured, clinically relevant fashion from the basics through to some more complex issues. The opening section of the book sets the scene, starting with a very readable chapter on the history of anaesthesia, albeit from an American viewpoint, from the introduction of nitrous oxide and ether through to the place of anaesthesia in modern-day medical care. This naturally leads onto a treatise on the scope of anaesthesia today touching on the multifaceted clinical aspects of the speciality and the development of safety and quality within anaesthesia.
From here, the reader is led into an excellent structured approach to learning anaesthesia, including some competencies to be acquired and how to set out a plan for the conduct of an anaesthetic. The plethora of sources of medical information is addressed, including an approach to critically evaluating that information and, in this age of electronic portability of information, some aspects of patient privacy. The main content of the book is divided into a further five sections which cover subjects which naturally fall together. These include pharmacology and physiology, preoperative preparation and intraoperative management, areas of special anaesthetic consideration, and the recovery period. The design, function, and use of anaesthetic equipment are covered in the appropriate clinical chapter.

The final section of the book outlines aspects of the skills and knowledge developed within the operating theatre which anaesthetists have applied to other areas of modern medical care, including critical care medicine, pain management, trauma care, and cardiopulmonary resuscitation. Importantly, there are chapters on the subjects of awareness under anaesthesia and quality of care and patient safety.

The focus of this book is very much on the planning and conduct of safe basic clinical anaesthesia, yet providing the reader with an overview of many of the other aspects of anaesthesia. Perhaps refreshingly, critical incident management is dealt within the context of the appropriate subject. By taking the underlying anatomical, physiological, and pharmacological principles and translating these into clinical practice, the book achieves its focus very well. Good use is made of simple clear and colour diagrams and illustrations throughout, to expand on the well-written text. The chapter on airway management is particularly well dealt with in this respect and the use of ultrasound-guided techniques benefits enormously from this approach. Initial reading of a section gives the basics of the subject addressed, yet re-reading will always uncover further useful and relevant detail.

Tucked away are several of these simple mental calculations many of us use in day-to-day practice to guide management such as in acid–base balance, respiratory management, and fluid administration. Each chapter ends with a short series of questions, ‘Questions of the Day’, the answers to which address the main points of that chapter, and each chapter provides a number of key up-to-date references. This book is obviously written from the standpoint of American practice and its layout does not map well onto the structure of the Royal College of Anaesthetists Fellowship examination.
Nevertheless, the contents are all there, excellently presented and this does not detract from its worth. As the title implies, it provides the basic framework for the conduct of safe anaesthesia upon which trainees can build.

The style of writing, presentation, and level of academic content commends this book to trainees at all levels, particularly as re-reading at different stages in training will always be rewarded. Indeed, anyone involved with training anaesthetists at any level, anywhere, will find something of benefit within its covers.
